Dr. Daniel Maman is a Harvard fellowship-trained and board-certified plastic surgeon specialized in cosmetic and reconstructive surgery of the breast, body, and face.

What is Breast Augmentation?

What choice is best for you? Here are a few important items to consider:
Implant material
Breast augmentation uses either saline or silicone implants, and different implant materials yield different results.
Silicone implants are the most popular option and are often preferred to saline for their more natural look and feel. They also usually require slightly larger incision sites than saline options, and ruptures can be more difficult to detect without an MRI.
Saline implant insertion sites are generally smaller, and ruptures are more easily detected. Some people also find saline implants to be less natural-looking on the average person and can sometimes show wrinkles or ripples over time.
Incision Location
Your incision location is decided based on your recommended implant type and size, your anatomy, and any concern about preserving nipple sensation.
Dr. Maman’s breast augmentation is typically done via an incision hidden within your natural breast fold or within the nipple. This approach is discreet, and also makes placement more direct and limits time spent during surgery.
Some patients, however, are better candidates for incisions along the bottom of the areola.
Because of increased likelihood of scarring and difficulty obtaining an accurate placement, our office doesn’t typically suggest using the armpit as an incision location.
Shape
Implants come in two standard shapes: round and teardrop. Round implants are the most common and provide the most upper fullness. Teardrop implants are less common due to their specific use and risk of malpositioning within the breast pocket. A thorough evaluation and discussion of your goals will be key in determining which shape may suit you best.
Implant Placement
In most patients, breast implants are placed between or behind the pectoral muscles (or subpectoral placement). For certain patients with specific anatomy, they may also be placed above the muscles behind the breast tissue (subglandular placement).
Several factors determine how to best approach your breast augmentation with implants. Subglandular implant surgery is more straightforward and has a less painful recovery. But there’s a tradeoff—these implants may look and feel less natural. An athletic patient, for example, may benefit from this approach due to the level of dynamic muscular activity they experience regularly.
On the other hand, subpectoral implants create natural-looking breasts with less risk of visible aspects of the breast implant. They are also good for patients worried about how their implants will hold up over time: implants placed behind pectoral muscles fall more naturally into the breast pocket and show less obvious rippling.

How do I prepare for Breast Augmentation surgery?
Preparing for breast augmentation involves several steps, including consulting with a board-certified plastic surgeon, discussing goals and expectations, undergoing medical evaluations, following pre-surgery instructions (such as refraining from smoking and certain medications), arranging for post-operative care, and ensuring a supportive recovery environment.
How much do you charge for breast augmentation?
Because we customize treatment for each patient, pricing can vary. During your initial consultation, Dr. Maman will conduct a detailed assessment of your goals. Afterward, you’ll receive an itemized price quote to help you make your decision.
Which type of implant—silicone or saline—is best?
99% of our patients choose smooth, round silicone implants. Often called gummy-bear implants, silicone implants feel and look more like natural breasts, last longer, and maintain their shape even after micro-ruptures.
The only benefits of saline implants are their comparatively lower cost and the fact that they can be placed with a smaller incision. Ruptures are also immediately detectable as the affected breast will flatten within hours.
Can breast augmentation be done using fat?
Natural breast enhancement—injecting fat to increase breast volume—provides more natural-feeling breasts. It also doesn’t introduce any material into your body, a concern for some patients. But this technique has several limitations.
Ideal candidates for this approach look for a small bust-size increase and have tight breast skin, no breast sagging, and adequate fat-harvest sites in your stomach/hips. The most significant risk with fat transfer augmentation is that fat can change position/texture or be reabsorbed by your body, leaving you with potentially misshapen or smaller-than-desired breasts.
How will I know which size breast implant is right for me?
Choosing the right implant size is both an art and a science, which is why we custom order implants for all our patients. Implants are measured in multiple ways, including width, volume, shape, projection, viscosity, and fill pressure. A careful balance of each of these factors, combined with your own goals, plays a critical role in achieving a natural, aesthetically pleasing breast augmentation.

My breasts are saggy. Am I a good candidate for breast augmentation?
Saggy breasts—ptosis in medical terminology—is caused by a combination of age, gravity, and pregnancy/breastfeeding. The result can be deflated breast tissue. Mild ptosis can be treated with a straightforward breast augmentation, but severe ptosis usually requires both breast augmentation and a breast lift, or mastopexy, to achieve the desired result. During your consultation, Dr. Maman can determine which approach meets your needs.
I just had a baby. How long should I wait before my breast augmentation? Can I breastfeed with breast implants?
We typically wait an average of six months after childbirth to perform plastic surgery. But because individual scenarios vary so widely, some patients wait only three months, while others wait up to a year.
It is important to remember that any surgery requires in-depth discussion and planning across several appointments. We recommend you come in for a consultation as soon as you are ready to gather information. This will give you time to prepare for your surgery without a long wait time.
Can I breastfeed if I have breast implants?
Will my nipple sensation change after breast augmentation surgery?
Most patients don’t experience change in nipple sensation—known as paresthesia—after breast augmentation surgery. While areolar incisions present a slightly higher risk of paresthesia, 90% of women who have this type of surgery regain full nipple sensation.
Will Breast Augmentation affect mammograms?
Breast augmentation can impact mammograms by making it slightly more challenging to detect abnormalities, particularly when placed above the muscle. For this reason, Dr. Maman usually puts implants below the muscle so that no distortion of breast imaging occurs. Experienced radiologists can perform specialized techniques to ensure accurate readings. It is essential to inform your radiologist about your implants before the mammogram to optimize the procedure. It is also important to adhere to a surgeon’s requirements for preoperative mammography before breast augmentation.
Is there an age limit for Breast Augmentation?
here is not a specific age limit for breast augmentation. However, it is crucial for candidates to be physically mature and emotionally ready for the procedure. Surgeons evaluate individuals on a case-by-case basis, considering factors like physical development, health status, and personal readiness. The FDA also makes cautionary recommendations for using implantable devices on younger patients due to lack of robust long-term clinical safety data. These are items you would discuss as part of the consultation with a board-certified plastic surgeon.
Can I still undergo Breast Augmentation if I have a family history of breast cancer?
Having a family history of breast cancer does not necessarily disqualify you from breast augmentation. However, it is crucial to discuss your medical history thoroughly with Dr. Maman. He may recommend additional screenings or consultations with a breast specialist or genetic counselor to assess your individual risk factors before proceeding.
Are there any risks involved with breast augmentation?
As with any surgical procedure, breast augmentation carries potential risks, including infection, bleeding, implant rupture, changes in sensation, scarring, and complications related to anesthesia. It is essential to discuss these risks with your surgeon beforehand and follow post-operative instructions diligently to minimize potential complications. Regular follow-ups with Dr. Maman are also crucial for monitoring and addressing any concerns during the recovery process.
What is the difference between Breast Augmentation and Breast Lift?
Breast augmentation involves increasing breast size or changing shape using implants or fat transfer. Conversely, a breast lift, also known as mastopexy, raises and reshapes breasts by removing excess skin and tightening the surrounding tissue, primarily addressing sagging or drooping without changing the size significantly. The two procedures can be performed concurrently as a mastopexy-augmentation to address both volume deficiency and skin laxity in one highly effective operation.
Surgical Procedure FAQs
Where will my surgery be performed? How long will it take?
Dr. Maman usually performs breast augmentation surgeries at an in-office, AAAASF-certified facility or an outpatient ambulatory surgery center (ASC). He also has operating privileges at Mount Sinai Hospital, Lenox Hill Hospital, New York Eye & Ear Infirmary, and Englewood Hospital. We work with you to select the best facility for your needs.
Breast augmentation surgery usually takes about two hours, including prep and recovery time. If you’re combining breast augmentation with other procedures, your total time in the operating room may be longer.
What will my incisions look like, and where will they be?
Choosing an incision site depends on various factors, including the value you place on nipple sensation, your anatomy, and the type/size of your implant.
Regardless of where your incisions are, they’ll be small, usually 1.25–1.5” long, and produce minimal scarring that’s easily hidden. Carefully following our standard scar treatment protocols also lessens scarring.
Because it’s the least visible site and provides the best surgical visualization, most of Dr. Maman’s breast augmentations are done through a breast crease incision under your natural breast fold.
For smaller-breasted patients with a well-defined areolar border and no naturally occurring breast fold, incisions can be made along the areola’s bottom edge. This approach allows for excellent implant positioning but does affect nipple sensation for a minority of patients.
Armpit incisions, while not a first choice, are most commonly used for placing saline implants. Because saline implants are filled after placement and come deflated, they can be inserted through smaller remote incisions before being tunneled into the breast pocket. We don’t recommend placing silicone implants using this method, as accurate placement is difficult and the incision results in a scar that may be visible in short-sleeved shirts.
Should my breast implants be placed above or below the muscle?
While above-muscle implant placement is easier and causes less pain, the result looks and feels significantly less natural than below-muscle placement. By contrast, subpectoral implants (implants placed beneath your chest muscles), result in natural-looking breasts and a realistic slope down to your nipple. Additionally, subpectoral implants result in less internal scar tissue, are less likely to drop with time, are less likely to produce a rippling effect, and are less likely to impact mammography.
Dr. Maman almost exclusively uses a dual-plane approach, placing the top 80-90% of your implant below your pectoral muscle and the bottom 10-20% directly below your breast tissue. This technique delivers the benefits of subpectoral implant placement and allows your implants to drop slightly over time for a more natural-looking result.
Can I combine my breast augmentation with other procedures?
Post-Surgery Questions
How long does recovery take?
Most of our breast augmentation patients find recovery easier than they expected. As long as you adhere to your activity restrictions and manage your pain medication well, you can return to work within a few days and resume lower body exercising (spinning, walking, etc.) in one to two weeks.
Workouts that require significant upper body involvement (yoga, weight lifting, core training) should be avoided for four to six weeks after surgery, and you shouldn’t lift anything heavier than 10 lbs for a full six weeks.
It’s important to remember that feeling better is not the same as being fully healed. Even if you aren’t in pain, observing the total recommended recovery time of four to six weeks will reduce your risk of bleeding and ensure your implants do not shift.
When can I go home?
Will I need special clothes after my surgery?
To ensure our patients have the best possible recovery, we provide custom-ordered post-surgical bras at no extra cost. Most patients also prefer to wear loose-fitting, button-down
tops for the first week or so, as taking clothes off overhead will be difficult at first.
Will I have to replace my breast implants in the future?
Possibly. The idea that breast implants need to be replaced every 10 years is based on a common misunderstanding of FDA implant recommendations. While it’s true that the risk of implant failure slightly increases after 10 years, implants can last as long as 25 years.
Typically, revisional surgeries are indicated either by a change in breast aesthetic because of aging, pregnancy/breastfeeding or implant failure. If your breast augmentation needs to be amended for aesthetic reasons, Dr. Maman works closely with you to determine optimal timing. In the event of an implant rupture, Dr. Maman and his team work with the implant manufacturer to determine warranty coverage.
Will I have to pay for follow-up visits after my surgery?
How long do I need to wait before exercising after Breast Augmentation?
Patients typically need to refrain from strenuous activities and exercise for about 4-6 weeks post-surgery to allow proper healing. Following Dr. Maman’s recommendations regarding the gradual resumption of physical activities is crucial to prevent complications and ensure optimal recovery.
